Neurological symptomatology: 2. Cerebral infarction presenting with a pure motor monoparesis

Eight cases with pure motor monoparesis (PMM) exhibiting only upper limb monoparesis due to cerebral infarction were studied using MRI diffusion highlighted images. Strong paralysis was observed at a distal site in all cases. Most responsible foci captured by the diffusion highlighted images were small foci of 15 mm or less. Multiple foci were observed in 6 cases. Internal carotid stenosis was suggested in 4 of 8 cases, and an embolic mechanism was suggested in 2 cases. It was emphasized that the responsible focus is captured by DWI as early as possible and an onset mechanism is examined to select the therapy for secondary prophylaxis of cerebral infarction.
