抄録/ポイント： 大部分のB細胞前駆体急性リンパ芽球性白血病(BCP ALL)は既知の主要な遺伝的サブタイプに分類できるが,BCP ALLの実質的な割合はその基礎となるゲノム異常に関してほとんど特性化されていない。したがって,RNA配列決定を用いて,1,223のBCP ALL症例のトランスクリプトーム景観を再分析し,描写するための大規模な国際的研究を開始した。14のBCP ALL遺伝子発現サブグループ(G1からG14)を同定した。8つの以前に記述されたサブグループ(MEF2D融合に関連するG1からG8,TCF3-PBX1融合,ETV6-RUNX1陽性/ETV6-RUNX1様)を除いて,6つの追加遺伝子発現サブグループを定義した:G9はPAX5とCRLF2融合の両方と関連していた。PAX5(p.P80R)とIKZF1(p.N159Y)の変異を伴うG10とG11;IGH-CEBPE融合によるG12とZEB2(p.H1038R)における突然変異;そして,それぞれTCF3/4-HLFとNUTM1融合によるG13とG14。小児BCP ALLにおいて,サブグループG2からG5およびG7(51から65/67染色体)は低リスク,G7(≦50染色体)と関連し,G9は中間リスクであったが,G1,G6およびG8は高リスクサブグループとして定義された。成人BCP ALLにおいて,G1,G2,G6およびG8は高リスクと関連していたが,G4,G5およびG7は比較的良好な結果を有していた。BCP ALLのこの大規模トランスクリプトーム配列分析は,BCP ALLの離散経路を反映し,疾患分類と予後層別化を形成する異なる分子サブグループを明らかにした。併用結果は,RNA配列決定がBCP ALLの臨床診断作業に導入されることを強く提唱する。Copyright 2019 The Author(s).
